Gwen, has over 30 years’ experience in the Aged Care and Disability Services having previously worked in both Aged Care and Hospital settings. Gwen then moved onto Aged Care and Disability Administration roles including Workforce Coordination, Supported Accommodation Manager and Assistant Community Care Manager.

Gwen’s qualifications include a Certificate III in Aged Care, Certificate III in Home & Community Care, Human Resource Management Process, Performance Management, Culturally Diversity, Elder Abuse and Understanding Dementia and Preventing Dementia with Wicking Dementia Centre University of Tasmania. Complex Needs Case Management. Death and dying Bathurst University, Registered Nurse trained but no longer registered.

When not working with clients Gwen loves spending time with family, and going camping with friends.

Home Care Packages – How To Apply

An ACAS assessment is required if you need to access any level of Home Care Package or entry to Government Funded age care basic in-home services.

In order to arrange an Aged Care Assessment (ACAS) you will need to contact My Aged Care Customer Service Centre or fill out the online form. Alternatively your own Doctor, Nurse or other Health professional can organise this for you on your behalf and with your consent.

My Aged Care will refer your application to an ACAS team that is in your Local Government Area (LGA). Eligibility is based on your care needs determined through a face-to-face assessment.
