The starting point is a referral to My Aged Care. This allows an aged care assessment to be arranged, which is used to confirm eligibility for government-funded services, including Support at Home.

We will help make sure the right information is submitted so the process is as smooth as possible.

By sharing this information, you consent to Local Guardians making a submission to MyAgedCare.gov.au on your behalf. An independent assessor will then be in touch to arrange an in-person assessment.
