Care Teams

Local Guardians operate care management services through local care teams. These services are also known as Case Management services (aged homecare packages) and Plan Management (National Disability Insurance Scheme).

Our teams operate under a person centred model, whereby every opportunity is given to the care recipient to make their own decisions and exercise choice and control.

We are an approved provider of Home Care to eligible clients under the Aged Care Act and a registered Plan Manager, under the National Disability Insurance Scheme.
