Adele Bromham
Adele grew up locally and has been working in the community sector since 2014 in various case management roles. Her qualifications include a Diploma of Community Services Management, Certificate IV in Mental Health and a Diploma in Nursing (Enrolled Nurse).
Adele works with Home Care clients across the ACT and has over 20 years’ experience in the community and health sector. She is passionate about people having choice and power over their own health needs. She understands the challenges with finding appropriate, trustworthy, reliable aged care support in the local community and has strong knowledge of the area and services available.
In her spare time, Adele likes to spend time with her family, and also enjoys participating in her local community, going for runs and escaping to the beach.

Aged Care Assessment – Canberra
ACAT assessment is an eligibility requirement to access Commonwealth-funded residential aged care, residential respite care, transition care and home care packages.
ACAT undertakes a comprehensive assessment of care needs for older people needing access to residential and/or home (community) care, and helps the elderly and their carers to determine what kind of support will best meet their needs.
Target group are older people, typically over the age of 65 or for indigenous Australians, people over the age of 50, or people who have an age related condition.
Often comprises of a doctor, nurse, social worker and occupational therapist, the team will recommend services deemed to be the most appropriate to meet care needs through a series of questions, and with the client’s consent will liaise with families/carers and health care providers.
Referral Procedure, by care recipient themselves or others:
- The care recipient, carer, family must be aware of and consent to the referral
- All referrals are to be made via MyAgedCare: 1800 200 422

Canberra Seniors Centre provides a wide range of activities including courses in different languages, exercise classes, painting, cards, and dance. Members can drop in and enjoy a cup of coffee or tea and lunches are also served regularly.

Council on the Ageing (COTA) ACT
COTA ACT is a local organisation working to improve the lives of older Canberrans. They coordinate several events aimed at supporting older Canberrans and offer a range of free programs and services to members which encourage older Canberrans to participate in our community. COTA ACT also currently manage the ACT Seniors Card program.

Belconnen Seniors is a group of lively and sociable people over the age of 50 who get together to have fun and learn new skills. They offer a range of fun and educational activities including craft classes, stretch and balance, old time and modern dancing, table tennis, singing/choir group, indoor bowls, cards, and coach trips when available.

The Tuggeranong 55 Plus Club offers a range of activities to stimulate the mind and keep the body active. The club encourages a positive focus on ageing and is open to anyone over the age of 50 in the community. The club runs weekly, bi-monthly and monthly activities, as well as ‘special events’.

Getting started is easy, once you’ve been approved for a home care package contact us and we’ll send you our simple, easy to read 4 page agreement. Once this has been completed and returned we’ll process it (generally within 1 business day) and be in touch regarding next steps.
Yes! Switching is easy.
- you’ll need to agree on an exit date with your existing provider, this might depend on your home care agreement, ie two weeks
- provide the date to Local Guardians and the contact details of your existing provider to ensure home care package unspent funds can be transferred across
